Carson Dermatology is committed to maintaining the privacy and security of our patients’ personal information.  Our practice takes your privacy seriously and we want you to know how we may use, share, and protect your information when you provide personal information through our website and/or mobile applications such as SMS (Short Message Service) communication.  Please note this privacy notice is in addition to our Notice of Privacy Practices that expressly addresses how your Individually Identifiable Health Information (IIHI) is subject to privacy regulations and compliance with HIPAA (Health Insurance Portability and Accountability Act).  

 

When you use our website or SMS communication, we may use the personal information that you voluntarily provide to us to better serve you as a patient.  If you choose to provide us with personal information, we will use your personal information to help us provide the service or information you have requested and/or simply to respond to your message.  Personal information may be collected from the following:

 

• Completion of patient registration forms through our website

• Completion of medical release forms through our website

• Online bill payment through our website

• Access to our patient portal

• Personal information voluntarily provided via email or SMS communication

 

When providing personal information through email or SMS, the amount of information you provide is completely voluntary.  Web forms submitted through our website, email, and SMS services are provided by third party vendors that comply with privacy standard assurances and security safeguards for compliance with HIPAA.  While our practice and other third party vendors employ physical, technical and administrative safeguards for the secure transmission of personal data, no transmission via the Internet or mobile phone carrier is completely secure from malicious compromise.  If you choose to submit personal information through our website, email, or SMS, you should understand that this transmission is voluntary and done at your own risk.  

 

You have a choice in your preferred method of communication(s). By providing your mobile phone number to us, your mobile number will only be used for healthcare related communication and will not be shared with third parties for any marketing purposes.  Standard message and data rates may apply depending on your wireless carrier.  You may opt-out of receiving SMS communications at any time.  To opt-out, reply “STOP” to any message you receive from us or contact our office directly.  Please note that opting-out will not affect other forms of communication you have requested from us such as emails or phone calls.

 

Carson Dermatology uses your personal information for internal purposes only.  We do not sell or disclose your personal information.  Your personal information is used to provide, coordinate, and manage your treatment of dermatologic and health care related services in addition to obtaining payment for the services provided to you.  Your personal information may also be provided as required by federal, state, or local law enforcement.  As previously stated, further information regarding the use of your IIHI may be found in our Notice of Privacy Practices.
